Questionnaires on Embodia

There are two ways to create consent forms on Embodia:

  1. Consent forms: to learn more about this type of consent form please refer to this help article. A quick summary is provided below: 
    • Tier 2 Embodia Members: patients provide consent by checking a box.
    • Tier 3 Embodia Members: you can choose to ask patients to provide consent in any of the following ways: by clicking a button, by typing their name, or by providing their signature by signing with their mouse or finger. 
  2. Questionnaires: patients consent by providing a digital signature.

In this help article, we will review how to use Questionnaires as consent forms.

How to create a consent form from an existing template on Embodia

You can use the Consent Form template available in the Questionnaires templates.

To do so, navigate to HEP >  Questionnaires > Browse available templates:

Then search for the template called 'Consent Form - General'. Click on it and then click Create a questionnaire from this template:

You can then modify the template to meet your needs.

 

How to create your own Consent Form

You can also create your own consent form from scratch by following the instructions outlined below.

From the Embodia dashboard navigate to HEP >  Questionnaires > Add questionnaire:

In the New questionnaire window, name your form, and set the type to Consent form.

Check the box next to 'Active' to make your consent form available for use, then click Submit:Creating consent forms on Embodia


Click on New item to start creating your consent form:

You can now add a New question or New instruction:

First, create a New instruction, where you add the static text (i.e., the content of the consent form):

Then create a New question. Set the Question type to Multiple answers or Single answer. Check the box next to Required, then click Submit. 

Your question can be something along the lines of "Check the box below to acknowledge that you fully understand the above consent statements and are entering into them voluntarily." 

Then click Submit:

Now add one answer to the question with a value of something along the lines of "I give my consent.":

You can now Preview your consent form.

You can also add a required question of type Free text > Short text that says: "Print your name below to acknowledge that you fully understand the above consent statements and are entering into them voluntarily.":

Preview your new consent form:

If you require a drawn signature for consent, you can add a required question that is a Drawing (#1 in the image below).

Select No background image (#2 in the image below).

Create your question text (e.g., "Please sign here to provide your consent") (#3 in the image below).

Optional: change the canvas size (the default is 800 x 450 pixels, however, we adjusted it to 500 x 250 in our example (#4 in the image below).

Preview your new consent form:

Once you are satisfied with your new consent form, click Back:Creating consent forms on Embodia

 

You will now see your new consent form in My questionnaires:
